Once we confirm availability in my practice, we are ready to take the next steps towards working together.

ALL families need to complete the basic intake forms included in the link below.

Then, families can choose to schedule their intake virtually or in person (billed at your self-selected sliding scale rate) OR complete the intake forms independently (at no cost). Families can anticipate spending 45-90 minutes with either option.
